Sure, you have to!

Since you didn't know about the address, I am assuming you don't know many other things about resume writing. Check out this website getresume.com (http://www.getresume.com) and download for free eBook on resume writing.

North American standard is different. You don't include too much personal info.
Your name, title if any, address, email, phone and that's it. All your contact info is needed.

The resume writing (http://www.jobagencies.ca/index.asp?p=4&sp=2) is not an easy thing to do if you want to do it well. you can check the Sample resume section too. It is on your right, just click the drop down menu for more resumes samples (http://www.jobagencies.ca/)

Address is important and other contact details for them to contact you for interview or job offer. IMHO, This is one way showing of sincere on whatever position you applying coz your giving your personal details.
Its better to check a sample resumes (http://malka-maxwell.com/) on how to make a raw resume into professional resume.
