Sure, you have to!

Since you didn't know about the address, I am assuming you don't know many other things about resume writing. Check out this website getresume.com and download for free eBook on resume writing.

Check the link in my signature. I have a section on resume formats and over 80 resume samples from professionals... all for free.

North American standard is different. You don't include too much personal info.
Your name, title if any, address, email, phone and that's it. All your contact info is needed.
